Submission of a consultation form or payment does not guarantee that a prescription will be issued. All clinical decisions are made solely at the discretion of the treating physician.

By using this website or purchasing services, you represent and warrant that you are 18 years of age or older, have the legal capacity to enter into a binding agreement, are located in a state where Dr. SkinnyShot™ is licensed to provide telehealth services, and that all information you provide is accurate, current, and complete.

Pharmacy fulfillment, shipping timelines, and product availability are subject to the policies of the dispensing pharmacy. Dr. SkinnyShot™ is not responsible for pharmacy delays, fulfillment issues, or product availability. Any concerns regarding pharmacy fulfillment should be directed to the dispensing pharmacy directly.
You agree to provide accurate and complete health history information during your consultation. You acknowledge that failure to disclose relevant medical conditions, medications, allergies, or prior treatments may affect your safety and the accuracy of your care. You agree to follow your provider’s instructions and to notify us of any adverse effects or changes in your health status.
